No you don't need to include your name as that kind o f stuff will already be on your application. We applied staright into our access course too and were told by college to follow this structure:
- Paragraph 1: What course do you want to apply for and why?
- Paragraph 2: What have you done on access/a-level that has helped to reach this decision? what have you learnt about yourself on this course?
- Paragraph 3: What work experience have you carried out?
- Paragraph 4: What interests, hobbies and activities do you have?
- Paragraph 5: What awards have you achieved? What sort of personality have you got that will help you on the course?
- Paragraph 6:What is your ultimate aim once you have completed the course?
Other things we were told to include were to mention how previous life experiences and studies will help on the course, what we knew about key issues and what we knew about the course.
Take a look at the personal statements thread in applying, mine is on there if you want to look. Everyone on my course who applied used this format and we all got offers of places first time round. Good luck and I hope it helps!
