thanks for your posts and for sharing stuff about yourselves that is obviously sensitive and incredibly personal.
I can understand your concerns, but think that the only people who can really honestly answer your questions are your prospective uni's occy health departments. I think you should give them a call, explain that you are enrolling and in the process of filling out the form and have a few questions. You do not have to give your name if you do not want to.

If these past situations are on your medical records then I would say that it is pretty much a given that you need to include all the information no matter what. But please don't let things worry you, you have obviously been through so much already, and dealt with many issues, that you must know that worrying about it now, is not going to change it!

I do believe that it really will not be a problem, occy health is about checking you are fit and well now at the start of your course, not what has happened in the past. However, as I say, you really should phone them.
