Hmm. tough one. These terms are springing to mind, but I am sure there'll be thousands more:
Social inclusion
Social exclusion
marginalisation / marginalization
inequality AND health
inequalities AND health
(did you know that for examples like the two above, where the ending of inequality/ies varies but basically means the same thing, you can put an asterix at the end, to get all possible variants o.e. inequal*

Will try and think of some more search terms whilst my brain is working. It's just getting that right combination of terms, and bingo.... you should get what you're looking for.
