In homebirths, the midwives here do mostly physiological 3rd stage. The only time they interfere is if there is an indication of a problem, and then they attempt herbs and homeopathy before pitocin IM. I do know that some midwifes still do CCT, at which point I'd be tempted to say that it's starting to be considered active management.
