It didn't at my uni, but just to put your mind at rest maybe you could ring the occy health dept at your uni and check with them.

To be honest I don't see how they can make it an issue, cos if you can't get the records you can't get them. But if you check you know for sure then.

how come you've got to fill out a form for dmu if you're going to coventry?

i wouldn't worry about it too much tbh -i am going to dmu...can't remember how far back i went -i think it just asked about depression etc and i ticked 'yes' and put the dates and what medication i was on. i had also just had a car accident and was seeing a physio at the time -all they did was ask for my consent to access my medical records -i had to sign a form and send it back that's all. they have okayed me anyway!! (and didn't cost me anything!)
i do remember filling out my vaccinations on one form or another (although may of been my work one i can't remember) but i asked at my doctors and they just printed it out for me! went straight down there and got it, they never mentionned money or anything.

I got an offer for DMU and they wanted my health form before they changed the offer to unconditional. In the end I chose coventry as my firm choice and birmingham and my insurance .

ahh cool! congratulations on your place! (i had my coventry interview scheduled for the same day as my dmu one so had to reschedule! never did get sent another date and by then i had an offer at dmu (just up the road from where i live so definitely my 1st choice as i have 2 kids to organise too)

i am only just getting to grips with this site so only just noticed the dates that it was originally posted haha

I'd just call the uni and ask how far back they want u to go. BCU asked for any periods of absense from work in the last 2 yrs but any oporations in my life if u see what i mean
