Default Health declaration form??

Hey everyone,
Ive just been sent a health declaration form from deMontfort uni....and i was just wondering how far back we have to go??
they ask questions about illness and operations and stuff but it doesnt say how far back you have to give information for!!do you have to go back to birth??lol?? or is it just during your working life-say 16 plus??
Im not sure how far back to give information for...so you could say im a bit stuck!!help!!

I am really sorry but i do not have a clue, What would do is get in touch with who ever sent the form and ask them how far back,
I am sure you would have to inculde any op's no matter how long ago I maybe wrong though

Any relevant info should suffice, visit your gp, with the form, they may charge you for it though.

Adult history really, unless any serious problems as a child.

Yes I would say the same, I suppose ops are the biggest thing-mainly back operations, I think I had to say all vaccinations I'd had, so I revisited horrendous school memories for those ones xx

I would probably fill in what you think is relevant i.e any ops youve had in the past. Probably also anything youve had wrong with you in the last three years or so (hopefully nothing lol) x

Thats why i was wondering how far back....as the only major thing is the last few years was pregnancy and a botched epiesiotomy.....which was nearly 4 years ago!!other than that nothing really...apart from a few childhood illnesses (chickenpox etc)

Just include those things you think are relevant.

They are looking for back probs and serious illnesses/conditions.

You have been well, I don't think you have anything to put really.

Mine contained my Asthma, and being admitted to hospital for it twice.

I hope they don't want EVERYTHING as I didn't have any jabs when I was a kid other than the TB one you get as a teenager! I was also a very sickly kid... never broke a bone, but had every ailment going!

I wouldn't include the pregnancy stuff, as its not really relevant to what they need, its more private for yourself, if you knowwhat I mean, just any jabs, rubella status , TB,mmr etc......

Will we need to have any japs we've missed? For instance.. I know fullwell I didn't turn up for my tetanus and dipth jab that I was sent for at 15!
