very cautiously, induction of labour, CTG in labour, fetal blood sampling as well as very regular obs on the woman, dimmed lights and we usually notify NICU and often have a paed at delivery, we give mag sulphate prophalactically as well, if any abnormal heart rate or womans bp hasn't come down with labetalol then we go for c/s without hesitation

Do you place concern for pre-e based only on BP or do you look at the whole picture? I'm starting to study this pretty in depth because it's starting to be a trend here where a women's bp can go up a couple of points at the end of pregnancy and the doctors schedule an induction. I always spilled a little protein in my urine, but I had one nurse absolutely freak on me. And, after waiting in the waiting room (with severe pelvic pain), my blood pressure had gone up a little. Nothing that concerned me, but the doctor tried to scare me. I have a little bit of background knowledge so I could see what he was doing, but to some, it freaks them out.

I'm right now studying nutrition and Dr. Brewer's findings. It is very interesting and as mw's don't have prescription privileges, they depend very heavily on excellent nutrition during pregnancy to avoid complications. I have seen very sick mamas who had true pre-e, but it's becoming something that is just so commonly diagnosed here, for really small things.

No we look at the bigger picture, Proteinurea ++ on 2 consecutive occasions, raised bp, epigastric pain, flashing lights, photophobia, diastolic of 90mmhg or more and now I believe systolic of 140mmhg but we have lots of women with systolic of round that at booking so I don't know how that one will pan out really. Or raised 20mmhg above the original baseline booking rate. We also do 24hour urine collection, as long as the woman is in otherwise comfortable health but symptomatic x

Oh that's wonderful. My bp normally is: systolic in the 120's and diastolic in the 80's. At the very end of my pregnancies (when I'm tired and sore and having a hard time just getting in and out of chairs), the systolic will occasionally get as high as 130. Never concerned me because while nursing, I know that people who are the least bit stressed can have slightly raised bp's. But the trend now is that women go into the doctors and have their bp go up just a couple of points and they are diagnosed as pre-e and are scheduled an induction or straight to c/section. Since I do a lot of education with pregnant moms, it's something that I'm starting to provide a lot of information on.
