COTW SMNET Catch of the week 12/12/11

    membranes intact?

    If so, great! Offer her a room. Encourage mobilization, hydration, light nutrition. What were her ideas on pain relief? What are the protocols in this unit re: birth pool? Is it 4 or 5cm where you are?

    Tell her how amazing she's doing and that it won't be too much longer until she and Josh gets to meet their baby.

    Obs all sound good. Would a 5bpm rise in FH be of concern after VE? Would you expect it to come back down soon after? Talk over birth plan, would you advise birthing ball/open pelvis positions as baby's position undefined in case it needs to turn?
    p.s how is she coping? +++ encouragement and reassurance x

    I would admit her,
    Have a bit of a chat about her birth plan, does she want any pain relief? To use the pool? Does she have a second birth partner she wants to call etc?

    Pulse up a bit but due to travelling in and contractions I wouldnt be too concerned. Recheck in 30mins?

    I wouldn't have thought that slight rise in FH would be a concern. Talk to cher about her preferences for the labour and 3rd stage, cord cutting e.t.c. and lots of encouragement and support. Does she want to use water for pain relief?

    We would write her as G4 P0 as well. If she had had a stillbirth we would write G4 P1-1. I looked after a woman this week who had had a 36 week fetocide (in Ghana) and one 26 week stillbirth. She'd just had her first livebirth so her parity was written G3P3-2.

    The contractions aren't lasting very long yet, do we know how long she has been contracting? As its her first birth it could still be a long while. How is she coping with the pain? If its not too bad then maybe a rest would be good or if its too painful get her to keep upright to get baby to come down and maybe get the contractions lasting longer. Guess it depends on what mums want to do.

    Parity is registerable birth - so if you have to register the birth it counts, post 24weeks you have to register it whether the baby was alive or not, so any past 24weeks would count, if they had a live birth at 23weeks - even if baby died shortly afterwards - that would also count but not if they died prior to birth

    A slight rise after VE is a good sign, the baby is reacting as you'd expect

    Membranes were intact

    No real rule on the pool, some do 5cm others aren't fussed - my personal idea is if they were 4cm dilated and contracting well by the time you fill the pool, chat to them etc they are probably 5 anyway
