    I think she seems to be making good progress. Is her bp a little on the low side? Guess that depends on her bp at booking.

    Re the 'leaking', I would be checking it was her waters and that it was clear.

    Pretty sure that the show should be pink or brown but not bright red, I could be wrong though so feel free to enlighten me.

    Measuring a little small for dates but head is fully engaged and in the light of the fact she has had a recent growth scan perhaps she just carries that way.

    Obs appear within normal limits. Ask about h/o leaking- when did it start, has she had a gush, is she wearing a pad. If so examine pad- are waters clear? If ?srom then I think I would want to do a speculum and take hvs and lvs as is policy at my trust, followed by a VE, all with consent.

    If the head is 0/5ths palpable and she is contracting why would u want to do a speculum and swobs? I'd more be thinking head on perineum!

    You're probably right, as I wrote I was wondering if that would be the best action but I guess my concern would be that her contractions disappear and then we're left with an unconfirmed SROM. Reading back through it does sound as if she is progressing well and hopefully VE will confirm. Obviously in practice with the woman in front of me I might be able to determine if she was definitely in established labour from observing her.

    So I guess my management would be, observe- does she appear to be in established labour, if so then VE.

    Can you check the amnio stick (i'm sure there's a different word for it but sure i've heard it called this before) on the pad or on her knickers rather than doing a HVS and if that is inconclusive then consider the HVS if necessary? Although it does sound like her waters have gone.
    How much roughly and did it smell different to urine? also colour?
    I know the smell thing is a bit gross but well, if it smells like urine then that's what it probably is and if not then hopefully her waters.
    Ask her to get on the bed just for a check.

    Show shouldn't be red though should it? What would cause it to be that colour?
    If she is progressing well, then surely just pain relief and 4 hourly obs? need to find out about unsuccessful pregnancy though, if baby is fully engaged, and no external signs of full dilation are there then definatley a ve?

    check the pad on the bed? Ask her to put on sanitary towel, to check. or would a ve determine if her waters had broke or were "leaking"? x

    In my (limited) experience if a woman SROMs on the bed, you can tell without having to do a VE or pop a pad on, if not obvious and she didn't already have one, I'd give her a pad and see what happened in next few contractions. Though a VE is most likely needed anyway so may just get on with that (with consent).
