Lactation is stimulated in the first instance by the dramatic fall in the placental hormones, oestrogen and progesterone, and then by putting the baby to the breast or, if this is not possible, by having skin to skin contact (Coad and Dunstall 2005). The release of the hormones oxytocin and prolactin is then stimulated from the posterior and anterior pituitary gland respectively (Rankin 2005). Prolactin is responsible for the production of the milk, manufactured by the acini cells, and is initially released in response to the baby suckling at the breast. Once feeding is established, milk production is maintained on a ‘supply and demand’ basis, the demand being the baby’s appetite. At the same time the hormone oxytocin, also known as the ‘love hormone’ is stimulated (Odent 2002) and produces the ‘let down’ reflex. In a study by Matthiesen et al (2001) this theory was proved by collecting maternal blood samples at 15 minute intervals and recording the movements of the baby. When oxytocin levels were analyzed they showed a distinct rise during “periods of increased massage like hand movements or sucking of the mother’s breast”. These benefits have been globally recognised and outlined by UNICEF and WHO in their Baby Friendly Hospital Initiative in 1991 (Hanson unknown date).
