Here are the questions and answers for those that missed it.... 1) Name the three stages of labour and briefly define them (extra marks will be awarded if you find a fourth stage!!) 1a) First stage of labour: from onset of regular contractions to fully dilated. Extra marks if latent or early phases are mentioned Second stage of labour: from full dilatation to birth of the baby third stage of labour: expulsion of the placenta extra marks awarded for 4th stage - transitional stage - the link between 1st and 2nd stage, usually when a woman says 'i cant do it!!' 2) Give two examples that show a woman is entering into the second stage of labour 2a) she becomes aggitated, cranky, irritated she withdraws into herself wants to bear down, or feelings of rectal pressure she wants to go home or says i cant do it anymore! 3) What is the normal range of a fetal hert beat, and name 3 methods of monitoring it 3a) 120 - 160 bpm Pinards, sonic aid/doppler, CTG 4) Name 2 ways which you can assess progress in labour without performing a vaginal examination 4a) observation of the purple line incidence of anal dilatation etc 5) Name 5 findings of a vaginal examination 5a) cervix - how dilated, thick/thin, position, the presenting part, station, membranes, etc 6) How many midwives should be present at a homebirth and why 6a) 2. One for mum and one for baby in case of emergency eg worst case scenario, PPH and baby needs resus as well 7) What is a partogram and name 5 things you would measure on it? 7a) a series of charted measurements used to assess progress of labour. FH, maternal temp, pulse, bp, contractions, pain relief, Liquor, cms dilated etc 8) Name 3 pharmalogical and 3 non-pharmalogical methods of pain relief which can be used in labour 8a) Entonox, pethidine, epidural TENS, water, hypnothreapy, reflexology, massage, birthing ball etc etc 9) What is an APGAR score? The APGAR score assesses the condition of baby at 1 min, 5 min and 10 mins following birth and gives a score of 0, 1 or 2 depending on condition of the baby. Baby assessed on respiratory effort, colour, tone, heart rate, response to stimulu 7 - 10 good condition 4 - 6 depressed condition requring some resusitation 0 - 3 severely depressed, requires immediate rescues and possibly ventialtion 10) what do you look for when checking a placenta and how big is it in relation to the baby? 10a) one sixth the size of a baby. Looking for chorion, amnion (or 2 membranes), 3 cord vessels, appearance, lobes, cord insertion etc etc Bonus question) What are the ischical spines and what are they used for? a) ischial spines are situated either side in the mid point of the womens pelvis and protude inwards about a centimetre. Some midwives feel for them when performing a vaginal examination, and others just imagine where they are. They are used to assess the station of the presenting part. Between -3 and -1 is above the spince, and + 1 etc is below the spines. When the presenting part is at the spines is also an indication of when a women wants to start pushing as the presentin part is sitting on the puedendal nerve as well!
