| that's basically it! active management means giving them pitocin (or I guess synto for you guys?) and then different methods to get placenta delivered. Here, they do CCT and lots of uterine massage (which hurts soooooooooooooo badly). Physiological means you just leave it alone, let mama nurse baby to stimulate contractions and let placenta deliver by itself.

| Yes. Syntometrine is injected IM after delivery of the anterior shoulder. This controls the bleeding and makes deliver of the placenta rapid (between 5 and ten mins post birth) Then CCT is used to remove the placenta, no maternal effort is involved. (I have just learned what the synthetic hormones are that constitute synto and what they do) My friend has seen two physiologicals and said they were great, I want to see some and hopefully will do in the third year. Hark at me, acting as if I know something! sorry

I much prefer the sound of nature being allowed to do this, after all surely if there are no indications for intervention this has to be best for Mum?

| Well, that's kinda my philosophy about it, too. I haven't really heard any strong arguments in favor of active management, though, so I try to keep myself open minded (very hard for me on some subjects!). It was always strange to me to see a birth where all through labor and delivery everyone sits back and encourages mom that her body knows what it's doing, etc, etc, then as soon as baby is born are in there getting that placenta delivered. Again, I need to do a lot more research on this before I feel confident on making definite opinions on anything.

| I am whole- heartedly for normality. Evidence suggests it has its uses for specific cases, but like the CTG it was never intended to be used generically. I am off to read Denis Walsh as I am sure he has some good stuff about active versus physiological. Will paste it into here as soon as

| Oh, thank you, that would be greatly appreciated. I've always had the mindset that interventions in themselves are not bad, evil, etc. Just the overuse of them makes them dangerous. There are times when an intervention made the difference in a big way, but I'm very much of the type to start small and work your way up, kwim?

| Anything you want to read about physiological, I'd say get Michel Odents books/articles, he's all for waiting away from the bonding experience leaving mum and baby to bond and for rooming in, he states watchful waiting is best. Should not give syntometrin with anterior shoulder though, I don't believe ppl should be practising the prophalactic use of syntometrin until the cord is clamped and cut (and till at least 30secs after birth/until pulsations cease)
