Default physiological third stages? do they happen in your trust?

This is virtually non-existent in our trust. All are offered syntometrine and advised to have it. All agree with the exception of one lady who was laughed at! And does everyone use CCT?

So what is the practice at your trust?

Women are given informed choice over active or physiological third stages.. They are given pro's and con's for each and then get to choice themselves.. I have done 4 physiological third stages.. Although most women still choice active third stages... CCT is used for active management! (I hate doing it!)

I know CCT is part of active management, some midwives class this as physiological, and this was written in documented home birth notes, postpartum. Obviously it is not and should be delivered by maternal effort.

Have you performed this in hospital births?

Yes, All my deliveries have been in the hospital setting.. 2 were done on labour ward and 2 in the birthing unit

In homebirths, the midwives here do mostly physiological 3rd stage. The only time they interfere is if there is an indication of a problem, and then they attempt herbs and homeopathy before pitocin IM. I do know that some midwifes still do CCT, at which point I'd be tempted to say that it's starting to be considered active management.

Yep we do some midwives as well as women have strong feelings about a physiological 3rd stage but either way all women are offered it..... I have had quite a few in my time, both at home and in the hospital. However the majority do seem to be active management, many women choose this route.....

Controlled Cord Traction. Wait for physilogical signs i.e small amount of bleeding and lengthening of the cord. Then careful tension applied to the cord.

Noooo problem, thanks for that, just like to know so I can try and remember things

So, is this when the midwife 'delivers' the placenta rather than it delivering naturally? Or have I got mixed up completely?
